The Nigeria Centre for Disease Control and Prevention has placed Lagos, the Federal Capital Territory, Kano, Rivers, and several other states on a high alert for Ebola preparedness following an official classification rating Nigeria’s risk of importing the deadly virus as high. In a sweeping national public health advisory sent directly to Commissioners for Health across all thirty-six states and the FCT, the apex disease control agency warned that the country must rapidly reinforce its epidemiological surveillance, isolation infrastructure, and infection prevention protocols. This preemptive mobilization comes as a highly dangerous strain of the virus continues to expand its footprint across critical corridors in the Democratic Republic of Congo and Uganda.

The urgency of the situation was significantly elevated after the World Health Organisation officially designated the current outbreak a Public Health Emergency of International Concern. According to the NCDC, this global declaration serves as a stark warning that Nigeria must secure nationwide operational readiness before any suspected case compromises its borders. While federal health officials emphasized that Nigeria has not recorded a single confirmed case within its territory, a comprehensive Dynamic Risk Assessment conducted alongside international development partners revealed that the country’s vulnerability remains critically elevated.

This pronounced vulnerability is largely driven by a combination of intense regional population movements, heavy volumes of international air travel, and highly porous land borders. Informal border crossings and historical trade routes linking West and Central Africa further complicate containment strategies, making passive monitoring insufficient. The scale of the threat is underscored by latest epidemiological data from the epicenter of the outbreak in the DRC and Uganda, where public health teams have logged 1,077 suspected cases and 247 fatalities, reflecting a severe case fatality rate of 24.6 percent. The NCDC noted that the demographic most affected by this current wave falls between the ages of 14 and 45 years, threatening the core of the regional workforce.

In response to these metrics, the NCDC structured a risk stratification framework to organize the country into three distinct preparedness tiers based on historical travel data, geographical exposure, and entry routes. Seated at the apex of this risk ladder are ten highly vulnerable jurisdictions: Lagos, the FCT, Rivers, Kano, Enugu, Borno, Akwa Ibom, Cross River, Taraba, and Adamawa. These areas have been explicitly designated as high-risk zones due to their status as major economic hubs hosting international airports, deep seaports, sprawling cross-border transit corridors, or heavy migratory densities.

While the federal advisory mandates that all thirty-six states maintain a baseline level of viral hemorrhagic fever preparedness, it explicitly demands that the high-risk jurisdictions accelerate their defensive measures with aggressive speed. Specifically, these frontline states must immediately scale up their active surveillance capabilities, verify the integrity of their laboratory diagnostics, and expand designated isolation capacities. Concurrently, the agency categorized a secondary tier of states—including Ogun, Nasarawa, Kaduna, Plateau, Kogi, Niger, Jigawa, Katsina, Bauchi, Ebonyi, Abia, and Bayelsa—as facing a moderate risk, requiring them to systematically review their local response mechanics.

A central concern driving this national alert is the specific viral architecture of the current outbreak, identified as the Bundibugyo Ebola Virus Disease strain. Public health officials warned that this particular strain currently lacks an approved, licensed vaccine or a definitive therapeutic treatment, leaving aggressive early intervention and strict supportive clinical care as a state’s primary line of defense. Compounding the challenge, the NCDC clarified that existing commercial Ebola vaccines and advanced monoclonal antibody treatments were engineered specifically to combat the more common Zaire strain. Consequently, these medical countermeasures cannot be relied upon to provide cross-protection against the circulating Bundibugyo variant.

Given these pharmaceutical limitations, the NCDC is focusing heavily on clinical education and early detection at the point of care. The advisory reminded healthcare professionals that while Ebola is highly transmissible through direct contact with infected blood, bodily fluids, contaminated medical equipment, or infected wildlife, it is not an airborne pathogen. However, the agency issued a stern warning regarding the diagnostic challenges presented by early clinical manifestations. The initial symptoms of the virus frequently mimic endemic tropical illnesses like severe malaria, typhoid, and Lassa fever, creating a significant risk of misdiagnosis.

Medical professionals across both public and private sectors have been instructed to maintain an exceptionally high index of suspicion and not wait for classic signs of advanced systemic failure, such as external bleeding, before initiating isolation protocols. The NCDC outlined a broad symptomatic profile for clinicians to monitor, ranging from sudden onset fever, profound fatigue, debilitating muscle pain, and severe headaches, to gastrointestinal distress including vomiting, diarrhea, abdominal pain, unexplained rashes, persistent hiccups, and eventual signs of cardiovascular shock. Any patient presenting with these symptoms alongside a relevant history of recent regional travel or potential exposure must be immediately isolated.

To ensure a seamless operational hierarchy, the NCDC has activated its National Emergency Operations Centre in alert mode, establishing a centralized command structure to harmonize federal assets with state-level health ministries. This unified platform is prioritizing the standardization of infection prevention and control practices, localized case management strategies, secure sample collection and transport logistics, and aggressive risk communication campaigns designed to prevent public panic. State governments have been directed to verify that every medical facility within their borders possesses functional contact tracing frameworks, readily deployable personal protective equipment, and secured holding bays.

Reflecting on the nation’s public health history, the NCDC noted that Nigeria maintains robust foundational infrastructure and experienced rapid response teams developed during its successful containment of the 2014 Zaire Ebola outbreak in Lagos. While these structural assets give the country an important advantage, the agency cautioned against institutional complacency. Federal health administrators concluded that while the designation of high-risk states forms a vital component of the nation’s early warning system, the ultimate success of Nigeria’s defensive perimeter will depend entirely on sustained state funding, political will, and unyielding vigilance on the frontlines.
